I, ________________________________________________________, as student,
and I/we, __________________________________________________, as parent(s)
of the student, do hereby agree to the following terms and conditions for
enrollment in the MAC P/SAT Preparation Program:
The payment of the tuition for this program is the obligation of the parent(s) and the
student if the student is 18 years of age or older. The MAC P/SAT Preparation
Program is an ongoing rolling admissions program. Tuition in the amount of
$2,132.00 is due in full upon the signing of this Agreement. This fee is for a period
of six (6) months from the date of the first class session. Upon the expiration of six
(6) months, there will, thereafter, be a $150.00 per month fee for any month that
the student remains a participant in the MAC P/SAT Preparation Program. Following
the initial six months, participating at any session during the course of a month
requires payment of $150.00 in advance, at the beginning of that full month. The
student may attend as many classes as the student chooses to do so during such month.
This fee will be levied whether the student attends one class in a particular month or more.
There are no exceptions. There is also required a one-time separate non-refundable $249.00
MAC ACTION INDICATORS a/k/a/ THE PROFILE and Analysis fee that is also due upon the
execution of this agreement. Payment for the MAC ACTION INDICATORS a/k/a/ THE PROFILE
and Analysis fee and the MAC P/SAT Preparation Program are to be separate, and a separate
check or cash payment in full is due for each. All checks for the MAC ACTION INDICATORS
a/k/a/ THE PROFILE are to be made payable to Dr. Jean Maculaitis, whereas checks for the MAC
P/SAT Preparation Program are to be made payable to MAC Testing & Consulting, Inc.
In consideration of the tuition paid under this agreement, MAC Testing will provide
to the student such materials and classes as MAC, at its sole discretion, deems
appropriate. These materials and classes may be changed at any time and from time-to-time
without prior notice.
All tuition and fees paid in connection with this Agreement are neither refundable
nor prorated according to the number of classes attended. When this Agreement is
executed, MAC has committed itself to reserving a seat for the student. Thus, MAC
has satisfied its obligation by reserving that seat, and the entire fee is earned
whether the student attends classes or not.
If a student is going to be absent from MAC for a period of thirty (30) days
or more, the student may request an extension of the six (6) month course, which
may be granted at the sole discretion of MAC. Any such extension shall be in writing
and shall specify the time period for which an extension is requested. There shall
be no exceptions.
It is the student’s responsibility to review the materials provided and prepare
for class. It is essential that students work through the MAC ACTION INDICATORS
a/k/a/ THE PROFILE, prior to attending classes at MAC. Special sessions are available
should a student wish to fill out THE PROFILE at MAC instead of at his/her home.
It is the student’s responsibility to attend his/her scheduled classes, to exhibit the proper
attitude, to pay attention in class and to record his/her attendance on the requisite
MAC attendance sheet. All sheets are to be retained in the designated location in the
entranceway of the MAC Center after the instructor signs them.
MAC may use the results of the student tests and other activities in connection with
any studies, articles, papers, books, presentations and/or other advertising MAC
deems appropriate.
Any invoices issued subsequent to the date of this Agreement that are not paid
within thirty (30) days of the date of said invoice shall be charged interest at
the rate of 2% per month. Any returned check shall result in a charge of $30.00.
This fee is in addition to all other sums due and owing. Reasonable collection
costs and attorney’s fees incurred in the collection of any monies due and owing
to MAC shall be the responsibility of the parent(s) and student if the student is
over 18 years of age at the time of entering into this Agreement. This responsibility
shall be joint and several.
No student may begin classes at MAC without the student and parent(s) having
signed and submitted this contract to MAC Testing & Consulting, Inc. on or
before the first scheduled session. There shall be no exceptions.
